Physicals are required for UW students who are employed within campus children’s centers.
For students who are unable to obtain these services through their own provider, The Office of Child Care & Family Resources will provide authorization for payment (to Occupational Medicine only) so that the student worker meets licensing requirements for child care employment. These fees are paid in full and are made possible through UW Madison student segregated fees.
Please note that the UW student should always use their regular health care provider first and seek this no cost option at Occupational Medicine ONLY when needed due to lack of other access.

For TB Tests and Physical Exams:
Authorization
Students seeking financial assistance for TB testing and Physicals will need authorization from the Campus Child Care Center Director.
The Director will add the student information to the Childcare Provider – Services Needed with Occupational Medicine document list.
Obtaining Services:
- The student will contact Occupational Medicine (OM) clinic directly to schedule an appointment.
- The OM staff member who schedules the appointment will inform the student of all pre-visit requirements to be completed in the MyUHS patient portal. These items are:
A) Completion of the two required Occupational Medicine Consent Forms:

- OCCUPATIONAL MEDICINE: Authorization for Release of Occupational Health Records
- OCCUPATIONAL MEDICINE: Privacy Notice and Consent to Treat

B) Submission of the TB Risk Assessment & Screening form.
C) Completion of the appointment’s pre-visit questionnaire.
Billing:
- OCCFR will pay for authorized student childcare workers for work related physicals ($40). If requested by a physician, OCCFR will also pay for TB testing ($15) for authorized student childcare workers.
- Billing must be prearranged by following the instructions above.
